Q: How do I begin to build my vital health record?

A:After you successfully log into the system, an Enrollment Assistant page will guide you the process of creating your medical record.

Q: Do I need to have any medical knowledge to use MyVitalData?

A:You don’t need any medical knowledge to use MyVitalData. If you have any difficulty with completing your information, you can call our member service representatives (888-683-3282) and they will answer any questions you have, or help you step-by-step to complete your record.

Q: How do I complete the information for my new MyVitalData vital health record?

A:NOTE: You do not need to complete all information at one time; however you should finish the personal information, medications, severe allergies, severe chronic conditions and emergency contact information as soon as possible. Begin by gathering the following together and have it available next to your computer:
  • personal information (name, address, phone number)
  • contact information for your personal physician, family member or friend – those who would be your appropriate emergency contacts
  • all your medications (the information you’ll need is on the bottle labels)
  • any information you may have regarding your medical conditions (if any)
  • any records you have of family medical histories
  • any records of test results, procedures, surgeries
Start with your Personal Information. As you complete each section, you will see a checkbox for sharing information with medical personnel. Check this box if you want medical personnel to have access to this section’s data. As you proceed, fill in as much information as you can. Remember you can go back and fill in more information at a later date using the edit record function.

Q: Can I store non-medical information on the site?

A:Yes. There is a section where you can upload files from your computer into your MyVitalData account.

Q: How do I add family members?

A:Assuming that you have already created an account for yourself and you can log into the MyVitalData.com site, go to “Administration and Add New Profile.” Enter first and last name of family member, then click Add button. After you have added a family member, go to “Select Profile” from left navigation panel. Select a family member from list and click Save button. The system should direct you to Personal Information page. Click Edit from this page and enter information as needed.

Q: How do I add additional information – allergies, medications, insurance, etc?

A:Go to the section that you want to add a record. From that section, hit the Add button to enter information. For instance, if you want to add Prilosec to your medications, then go to Medication page. Click Add to enter Prilosec information.

Q: How do I edit information - allergies, medications, insurance, etc?

A:Go to the section that contains the record you want to edit. From that section, look for the record that you want to edit. Hit the edit button for that record and enter information. For instance, if you want to edit Prilosec from your medications, then go to Medication page. Look for Prilosec, click edit and then change the information you want.

Q: How do I delete information in my record?

A:Go to the section that contains the record you want to delete. From that section, look for the record that you want to delete. Hit the delete button for that record and click OK to delete item. For instance, if you want to delete Prilosec from medication, then go to Medication page. Look for Prilosec and delete it.

Q: How do I upload images? What is the maximum size and how many images can I attach?

A:Log into www.MyVitalData.com. Click on “Administration and Upload Pictures” from the left navigation panel. Click Edit button. Click Browse button. Locate the image file you want to upload. (Only image types JPEG, GIF, PNG or BMP can be uploaded.) The image size should be approximately 100x100 pixels and less than 30 KB. You can upload as many image files as you like as long as they are less than 30 KB and you do not exceed your maximum storage of 10 MB. (You may purchase additional storage, if needed. Go to Attachment from the left navigation panel and click "Buy More Space.")

Q: What is a MyVitalData Card?

A:After you have completed entering your vital data, please print out your MyVitalData ID Card. You can print your MyVitalData Card directly from your vital health records. Make sure your printer is turned on. Log onto the website and access your records, then go to "Administration", “Print Records" and download the MyVitalData Card. This card contains the critical information that an emergency responder will need to access you vital health records in case of an emergency. You should carry this card in your wallet next to your driver’s license and/or ID.
